*The QIS process training is a two-day, 11 hour course which will cover each of the two phases and the nine tasks.

Day 1 – Tuesday, October 20, 2009


Across America CMS continues to transition to the Quality Indicator Survey (QIS) process. The QIS process incorporates a comprehensive, systematic and automated approach to review all regulatory areas. Through the use of sophisticated computer software, surveyors determine a facility’s Quality Care Indicators (QCIs) and subsequently compares this facility’s outcomes to the nationally established thresholds. Each QCI that is greater than the CMS threshold requires an in-depth surveyor investigation to determine facility compliance.

The Fundamentals of What You Need to Know will guide participants through CMS’s two-phase multi-task process with a comprehensive review including off-site preparation, entrance process, the initial tour, sample selection, investigative interviews, observations and record reviews, facility level task reviews, QCI determinations, surveyor investigations, preliminary deficiency determinations, and deficiency outcomes. This will be a tremendous opportunity to learn the Quality Indicator Survey process from one of the nation’s leading QIS educators and to receive accurate and up-to-date information on the survey process changes.

8:30 - 10:00 a.m. Review of the 162 Quality Care Indicators
10:00 - 10:15 a.m. Break
10:15 - 12:00 noon Procedural tasks including off-site preparation, entrance and on-site activities, initial tour and resident sampling and the use of MDS data
12:00 - 1:00 p.m. Lunch
1:00 - 1:30 p.m. Procedural tasks including resident interviews, observations and record review, family interviews, staff interviews, comprehensive record reviews and facility level tasks
1:30 - 1:45 p.m. Break
1:45 - 3:00 p.m. Surveyor investigative protocols, triggered facility tasks, medication review, survey data gathering, deficiency determination, exit conference


Day 2- Tuesday, October 21, 2009


The QIS software identifies the care areas to be investigated in Stage II by determining a facility’s Quality Care Indicators. CMS has an extensive list of procedural guidelines for surveyors to use for the investigations including the nursing home regulations and the state operations manual, CMS’s published guidelines which are a collection of detailed protocols and the QIS process’ Critical Element Pathways. The Critical Element Pathways are a systematic and consistent way to review a variety of care issues and services. CMS has developed these additional tools for surveyors in order to enhance the investigative process.

Beyond the Basics … Investigations, Processes and Outcomes will explore the processes and techniques used by surveyors to investigate and validate the existence of non-compliance. There will be a comprehensive evaluation of the Critical Element Pathways along with the CMS’s state operations manual and regulatory guidelines that are commonly used during the Quality Indicator Survey. Mr. Daily will share innovative, proactive, realistic approaches to identify your facility’s survey weaknesses and how to prepare for the new survey process.
